Clinicians use an explicit prompt that defines what the system should generate and what it should not. This keeps the output aligned with clinical intent and avoids the system “deciding” what matters.
Mandatory clinician review and accountability
Notes are always fully editable, and review is an assumed part of the workflow. The system is designed to support clinicians, not replace judgement or responsibility.
